For Relief by Widows of Confederate Soldiers or Sailors, under Act approved February 13, 1891
THE STATE OF ALABAMA
County of Butler
Personally appeared before me, [illegible], Mrs. L A Echols, who, being duly sworn, upon oath, says that her husband John W Echols was a Privet Solider in company [I?] of 1 first Regiment of Alabama; that he came to his death from wounds received or disease contracted in the service of the Confederate States, and that his death occurred during the late war or within five years thereafter; that she has not since re-married; that she was a resident of Alabama on the 13th day of February, 1891, and is a resident at the date of this application; and that her taxable property does not exceed four hundred dollars in value.
SWORN TO and subcribed before me, this 3 day of April 1893

Note that the key details are handwritten and often difficult to read. I believe it says "Privet Soldier" (possibly a misspelling of "Private Soldier"?) in company "I" (looks like a backwards 6 due to curl). In addition, the witness' name is difficult to read.
